Welcome to the St. Alphonsus Catholic School Family!  We are so excited you have decided to enroll and are happy to guide you through the Enrollment process below. 

If you have specific questions, please contact the School Office at (414) 421-1760.  Please note that the building is closed for tours through the month of July, and will reopen in early August.

New Student Enrollment

To enroll a new student for 3K through 8th Grade, please mail in or drop off at the school office the following forms:

• Completed New Family Enrollment Form.
• Non-refundable Registration Fee of $75.  This can be paid in cash or check made payable to St. Alphonsus Catholic School.
• Immunization Record
• Birth Certificate
• Baptismal Certificate (if applicable)
