Health Forms

Use our PowerSchool student information system to access health forms.  Instructions for how to access those within PowerSchool are linked to the right.

Students MAY NOT self-administer medication.  All medications are stored and administered through the school office.

All newly enrolled students also need to submit a Record of Immunizations to the school office.  You may email this to jramos@st-alphonsus.org.  Returning students do not have to submit this record. 

If you need the prescription medical form for your provider to sign, you may download it below.  In addition, if you plan to send ANY form of over-the-counter medication to school with your child, including cough drops, hydrocortisone cream, etc., we need to have a Non-Prescription Med Form on file.  All medication is stored in the school office and documented.
