Health Forms

Use the links to the left to access our School Health Forms.  ALL students need a Confidential Annual Student Health Questionnaire on file each year.  If your child will need prescription OR non-prescription medication administered at school, these additional forms must also be on file. 

Students MAY NOT self-administer medication.  All medications are stored and administered through the school office.

All newly enrolled students also need to submit a Record of Immunizations to the school office.  You may email this to giovanna@st-alphonsus.org.  Returning students do not have to submit this record.
